Yale New Haven Hospital Homeless Patient Screening Tool

Yale New Haven Hospital asks the following housing-related questions to determine whether patients are homeless. Additional sample questions asked by various hospital and health systems are available on the National Health Care for the Homeless Council’s  website in its policy brief Ask & Code: Documenting Homelessness Throughout the Health Care System .

  • Where have you been living during the past two months?
  • Is this reliable housing that you own, rent, or stay in as part of a household?
  • Are you able to return and stay there following discharge?
    If yes, are you able to receive a visiting nurse there?
    (If no, discharge planner should consider whether medical respite is needed post-discharge.)
